Lenawee Community Mental Health Authority is currently recruiting a full time Integrated Care Manager. The candidate will:
Coordinate health plans for individuals who have been identified as having chronic health problems as well as mental illness and/or substance use disorders, or developmental disorders;
Assist an interdisciplinary team with information and referral, education about mental illness, substance use disorders, and developmental disorders, and is a liaison between the primary health clinic and mental health and substance abuse providers;
Provide consultation and crisis stabilization services for urgent and emergency mental health and/ or substance use situations that present in the primary care population served in the clinic;
Assist in the development of wellness education.

Master’s Degree from an accredited program in Social Work; certification or licensure in the State of Michigan is required.
