Assists patients and their families with personal and environmental difficulties, which predispose illness or interfere with obtaining maximum benefit from medical care. Collaborates with the Program Director and the Interdisciplinary team (IDT) to ensure timely progress through the system and coping with chronic kidney disease (CKD)and end stage renal disease(ESRD). Performs core functions of assessment, advocacy, education and information, referral and linkage, coordination, and counseling and crisis intervention for assigned patients. MSW is required, LSW or LCSW
A Master’s degree in Social Work (MSW) required.
MINIMUM WORK EXPERIENCE:
Social services experience in a health care setting required. REQUIRED LICENSES, CERTIFICATES,
Licensure by the State of Illinois as a Social Worker (LSW) or Clinical Social Worker (LCSW) required.
