The Aging Life Care Manager will carry a caseload of clients while also providing clinical supervision for Home Care Aides. Responsibilities include meeting with clients and their families to assess the current care situation; developing a comprehensive care plan that addresses clients’ needs and goals; arranging, directing, and monitoring care; acting as a client advocate to ensure quality of care; and providing placement consultation services as needed. Additional responsibilities will include supporting the company’s business development activities by identifying and following up on potential customers and referral sources and participating in outside marketing activities.
