What would your day involve?
Create member-centered care plans and manage community-based care and services to improve the social, health, safety, and mobility outcomes of our members
Coordinate services between members and providers in the community
Collaborate with an interdisciplinary team
Inspire, support, and advocate for our members within the county
Provide wellness and prevention education to promote wellbeing
Implement risk-reduction and prevention strategies related to health, safety, and function
Evaluate and implement social service interventions based on assessment findings

Monday – Friday, 8:00 – 4:30 schedule. No weekends, holidays, or on-call shifts!!

What are we looking for?
Bachelors degree in social work or human services related field
One (1) year of experience working with frail elders and/or adults with disabilities
Strong critical thinking and proven assessment skills
Desire to collaborate to provide the best possible care for members
Adaptability and flexibility to manage through change
Additional Salary Information: Salary is based on experience
