Assumes responsibility for a collaborative process which facilitates implementation of the patient’s plan of care from pre-hospital to post-discharge. Conducts accurate assessments of patients to determine potential for discharge planning interventions and level of care based on approved criteria and standards. Plans transfers to nursing facilities, rehab, psych, and other care facilities. Provides non-medical patient care encompassing; assisting patient and family with adaptation to illness and/or disability and treatment; psycho-social/other discharge planning; facilitating the use of services offered by other hospital departments and community agencies; making social evaluations; evaluating psycho-social functioning; mental status examination; crisis intervention and intervention around specific high-risk disease and/or psycho-social problems. Works collaboratively with the Case Manager II (Registered Professional Nurse) on high-risk cases and issues of non-compliance. The Case Manager I and the Case Manager II may share some of the elements of discharge planning (i.e.; Case Manager II completes the PRI and Case Manager I completes the Screen, patients refusing to accept needed services, and leaving against medical advice, etc.). Ongoing assessments and reassessments are critical to the role. The caseload of a Case Manager I will change throughout the course of a patient's hospital stay. The same patient might be assigned and then unassigned to the Social Worker as circumstances change.
A Master’s Degree in Social Work ( MSW) , from an accredited institution and current licensure (LMSW, LCSW) or certification required by state for the specific field. Minimal 3 years’ experience of social work experience in a hospital or community-based setting, including experience providing individual, couple and family support. PRI Screen preferred, CCM certification preferred. Licensed Master Social Work (LMSW, LCSW) or certification required by state for the specific field. Membership in a professional organization preferred. Qualities that include effective written and verbal communication skills, decision-making and problem solving skills, and comprehensive assessment skills.
