As Coordinator for our WICY Program, you will:
Conduct intake assessments and continual reassessment of patients (HIV+ or at-risk youth, children, pregnant mothers and families)
Develop care/service plans and ensure effective multidisciplinary service delivery
Provide follow-up care, referrals and advocacy for patients, including linkage to community-based agencies with pediatric, adolescent or family care programs
Provide medical case management that also encompasses psycho-social needs
Work closely with the management team of the program
Determine patient and family needs, strengths and possible barriers to care
Clarify the spectrum of services available to individuals and families
Oversee patient testing and counseling
Follow hospitalized children in the PATH Program
Maintain documentation, serve on committees, and participate in performance improvement initiatives
Visit HIV+ mothers on the hospital floor and provide social work discharge services
Qualifications for this position include:
Master’s degree in Social Work
Current NYS License or ability to obtain within 1 year of hire
Excellent communication skills and preferably bilingual in Spanish or Creole
Understanding of the social, emotional and mental health issue that impact HIV awareness and care
Minimum two years’ experience in an HIV clinic setting for women, infants, children or youth
