This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Learn more The primary responsibilities of the Senior Social Worker MHICM are to provide intensive case management services, continuous quality improvement activities, evaluation/consultation, psychosocial assessments, individual therapy, placement services, and referrals to various community agencies and hospital services to MHICM patients. Provides intensive case management and treatment services to assigned veterans and significant others, consistent with the goals and methods of MHICM and the VA Mental Health Initiative and with all relevant Medical Center policies and procedures. Participate in team staffing regarding patient selection, determination of treatment plans and goals, progress evaluation, discharge planning, and after care. Work assertively and consistently with VA staff and inpatient teams to assure continuity of care by providing consultation regarding clinical needs, treatment plans, and case management services for assigned patients. Works assertively to promote appropriate treatment, rehabilitation, and expeditious discharge of assigned patients, consistent with program goals and with the individual treatment plans. Completes clinical documentation according to MHICM and Medical Center standards; this includes but is not limited to electronic progress notes, clinical progress reports, the Initial Assessments and Multidisciplinary Treatment Plans. S (he) will be responsible for completion of required clinical records as well as MHICM forms for evaluation and monitoring of service delivery and for the MHICM Unit Manager, Outpatient Clinic and staff at Northeast Program Evaluation Center (NEPEC). Coordination of work with other clinical programs and product lines that share the care of assigned MHICM patients, including but not limited to Substance Abuse, Recovery and Supportive Employment programs. In pursuit of programmatic and treatment goals, maintain effective working relationships and other professional and administrative personnel both within the Medical Center and in the community. Some of the duties include, but are not limited to; Responds to consults or referrals sent to the MHICM program by independently interviewing the Veteran utilizing questionnaires and other measurement devices while assessing the Veteran's emotional, functional, and psychological condition. Functions as a case manager for Veterans in the MHICM program, performing duties to facilitate independent community living by coordinating mental health and community care. Participates in developing a treatment plan for each MHICM Veteran with measurable goals outlined based on relevant psychological, medical, and psychiatric information with the principles of treatment matching. Serve as the major consultant for information on negotiating the complex processes Veterans encounter when interacting with government and community agencies. Independently conduct individual and family clinical interventions to facilitate the identification of substance abuse, psychiatric illness, and social needs as treatment issues. Independently assess the MHICM Veterans' learning needs regarding their substance abuse, psychiatric and medical disorders, and the treatments and medications required to incorporate teaching in the treatment. Independently conduct home visits and community visits to MHICM Veterans to maintain and encourage community living. Collaborate with community agencies where MHICM Veterans reside to facilitate community living in the least restrictive environment and to provide support services for these agencies. Other duties as assigned. Preferred Experience: Experience in Mental Health Social Work, experience in community based work, experience working with persons who have serious mental illness. Must be able to operate a GSA vehicle to conduct home visits and transport patients. Work Schedule: Mon-Fri 8:00 AM to 4:30 PM. The selectee may be required to work or travel between locations.
