This position is located in Social Work Service of the Bay Pines VA Healthcare System at the Lee County Health Care Center, Cape Coral, FL. The duties and responsibilities are carried throughout the HealthCare Center including all clinical and other patient care areas involved with the services. The Bay Pines VA Medical Center is a general medical and surgical facility, classified as a Level 1A Complexity, providing a full range of patient care services. Social Work Coordinators may be the sole practitioner in this specialty at the facility and typically provide direct patient care services in the program area. They manage the daily operation of the program, develop policies and procedures for program operation, and prepare reports and statistics for facility, VISN, and national use. They are responsible for the program's budget. The incumbent administers information and analytical systems to evaluate and enhance the quality of service provided to Veterans in the Redefining Elder Care in America Project (RECAP). Develops and implements information systems to track service activities including visits, referrals, inpatient days, patient demographics, equipment utilization and patient satisfaction. Develops implements and evaluates clinical guidelines and protocols to establish appropriate utilization of services and referral to Non-Instructional Care Programs. The incumbent interviews Veterans and their family members/significant others to establish facts about the Veteran's situation, presenting problems and their causes, and the impact of such problems on the Veteran's functioning and health as part of a comprehensive psychosocial assessment and focuses on the patient in the context of family and community by integrating an assessment of living conditions, individual and family dynamics, and cultural background into the patient's plan of care. Interprets and explains VA's treatment and benefit programs. The incumbent reviews all data, subjective and objective, and makes a clinical assessment, identifying needs and strengths. Effectively uses professional skill, objectivity and insight. Uses advance clinical training, insight and experience to interpret data and to identify viable treatment options. Assesses high risk factors, acuity and need for services. The incumbent participates in the assignment of Veterans to treatment teams and programs. Consults with other specialists in planning treatment for Veterans with co- morbidities. The incumbent Is responsible for clinical Social work practice in the specialized social work program to which assigned, including promotion of Social Work services as a key component of treatment. The incumbent performs a wide range of services and provides technical assistance to State, local, and private agencies, and universities. Technical assistance provided is based on monitoring activities and program reviews, and the development and analysis of information. In addition, the incumbent uses sound judgment in the interpretation of trends and patterns; the application of guidelines; and the recommendation of program enhancements. The incumbent performs program analysis, development, review, research, design, and contact work as possible with various agencies (public, DOD, state, etc.). The incumbent ensures adequate program coverage and makes changes accordingly. The incumbent utilizes all forms of electronic means of communication with Veterans and caregivers to include Video on Demand in order to Telehealth to increase the Veterans access to care. The incumbent ensures that work is completed in a timely manner and reflects quality documentation in accordance with Service, Medical Center, VA and The Joint Commission policy and standards. The Coordinator may also be assigned or requested to: Advise Service Chiefs, MH Directors, Executive Leadership, or VISN Liaisons Serve as liaisons to the facility committees and councils (such as QI/QM, RCA, Consumer Councils, or family support, etc.). Provide monthly, quarterly or annual reports on implementation progress as requested. Participate in conference calls to coordinate medical center or network and implementation of VHA policies and procedures related to the program assigned.
