Learn more The VA Palo Alto Health Care system is currently recruiting for a GS-12 Social Worker (Program Coordinator) for the Social Work Service in the for the Tele-Specialty Hub. The incumbent will be responsible for the development and implementation of the Tele-Specialty Hub at VAPAHCS in conjunction with a multi-disciplinary team. Responsibilities include development and monitoring of performance metrics, creation of policies and procedures to ensure timely and effective medical care is provided, direct social work services to Veterans and their families referred to VAPAHCS for specialty medical services, and care coordination and navigation for Veterans and their families utilizing VAPAHCS. Services will be provided virtually through VVC or other virtual modalities. Incumbent develops and implements methods for measuring effectiveness of social work practice and services within the Tele-Specialty Hub, utilizing outcome evaluations to improve treatment services. Incumbent demonstrates the ability to design changes based on empirical findings. Incumbent maintains accurate statistics and prepares reports within specified deadlines as required, including length of stay of patients requiring admission to VAPAHCS for specialized medical services, admitting diagnoses, psychosocial issues delaying discharges, disposition, etc. to track trends related to barriers to effective and timely discharges. Incumbent may provide direct clinical supervision for unlicensed staff working towards social work licensure or certification; guidance, and/or consultation to colleagues and students within Social Work Service. The incumbent renders professional opinions based on experience and expertise and role modeling effective social work practice skills in medical specialty services. The incumbent may be responsible for various control points assigned to the Tele-Specialty Hub. The incumbent manages and controls the use of data related to the Tele-Specialty Hub regarding day to day functioning and program evaluation. Incumbent plans, organizes and directs administrative, operational, and patient-related activities for the Tele-Specialty Hub. Incumbent participates in Tele-Specialty Hub program planning and developing relationships between VAPACHS and other VA healthcare systems within the VISN. In conjunction with medical specialty services located at VAPAHCS, incumbent promotes exploring strategies for developing closer collaboration with other VA healthcare systems to address the psychosocial, social and medical needs of veterans receiving care virtually or who may require lodging and transportation assistance to receive the medical care in person at VAPAHCS. This includes collaborating with various departments and services including Defenders Lodge, Fisher House, Business Administration Service, multiple specialty medical services within VAPAHCS as well as at other healthcare systems within the VISN.
