Learn more The VA Palo Alto Health Care system is currently recruiting for a GS-12 Social Worker (Program Coordinator) for the Social Work Service in the Hospice Care Center. Incumbent serves as program coordinator and is administratively responsible for coordinating admissions to the in-patient hospice and palliative care unit and providing specialized clinical Social Work Services to Veterans and families facing end-of-life care. Incumbent serves as point of contact/liaison for all inquiries regarding the Hospice Program. This may include stakeholders (e.g., veterans, families, community hospice programs, community hospitals, Veterans Service Organizations, and Congressional inquiries. Incumbent manages the daily operations of admissions to the program, directs patient care services in the program area, works with Hospice Administrative Team to develop policies and procedures for program operation and prepares reports and statistics for facility, VISN, and national use as needed. Develops a system to identify hospice eligible veterans served by the facility and community, thus ensuring access to services. This may involve use of VISTA and CPRS records, referrals from community hospice programs, and community hospitals. The incumbent manages and controls the use of data related to these programs regarding day to day functioning and program evaluation. In conjunction with the Hospice Administrative Team, Incumbent plans, organizes and directs administrative, operational, patient and personnel activities for the service. Incumbent provides supervision for social work fellows, social work interns and may provide clinical supervision for social work licensure or certification. As the Program Coordinator, Incumbent holds a high degree of responsibility for program operation, develops protocol and standards for referrals and for following up on referrals to community hospice agencies. Incumbent serves as point of contact/liaison for all inquiries regarding the hospice program. Incumbent coordinates referrals and admissions to the in-patient hospice and palliative care unit. Referrals come from acute care hospital, VA Community Living Centers (CLC), outpatient clinics, VA community-based programs (HBPC, MHICM, CNH), community hospitals, skilled nursing facilities, residential care facilities and home hospice agencies. Incumbent is responsible for screening and interviewing prospective Veterans for eligibility for the Hospice Care Center. Ensures prospective patients are appropriately enrolled in the health care system and works closely with Health Administrative Services (HAS) admissions and eligibility department. Incumbent gathers appropriate medical records from referring facilities to assist in evaluation of appropriateness of referred clients. Incumbent serves on multidisciplinary teams working with Veterans dealing with end-of-life issues, terminal or chronic illness and diagnoses; provides services in and hospice care unit. Incumbent demonstrates advanced level practice skills and abilities, with specialty focus in end-of-life care. Incumbent develops program supports within the health care setting to meet the complex care needs for hospice Veterans and their caregivers. Incumbent provides a consultative service to other areas of the hospital seeking input from palliative care specialists. Incumbent incorporates complex multiple causation in differential diagnosis and treatment of Veteran, including making psychosocial and psychiatric diagnoses within approved clinical privileges or scope of practice. The incumbent develops and implements methods for measuring effectiveness of social work practice and services in the Palliative Care specialty area, utilizing outcome evaluations such as the Bereaved Family Survey to improve treatment services. The incumbent demonstrates the ability to design changes based on empirical findings. Incumbent demonstrates an ability to evaluate practice, services, program effectiveness, cost effectiveness, and veteran and family satisfaction. Incumbent strives to reach optimal provision of quality of care, continuity of services, and the most advantageous use of resources to benefit veterans.
