The Greater Los Angeles VA is recruiting for a Senior Social Worker (Coordinated Entry Services Specialist) the Community Engagement & Enrichment Services (CERS), Housing & Urban Development-Veterans Affairs Supportive Housing (HUD-VASH) program in Oxnard, CA. S/he monitors the VA integration into the Coordinated Entry System and is able to access VA medical, mental health, substance use, other specialty clinical in addition to community providers, to collaborate in the care of Veterans. The CE enables VA homeless programs to fully collaborate with the community, including Continuum of Care (CoC) and community partners, in the coordinated entry efforts in that community. The major duties and responsibilities include but is not limited to: Ensuring continuity of care for homeless Veterans between VHA and community-based programs, as the Veteran transitions from homelessness into programs such as HCHV Contract Residential Housing (CRS), Residential Rehabilitation and Treatment Program (RRTPs), and Grant Per Diem Programs (GPD), as well as directly to permanent housing through the HUD-VASH Program, the Supportive Services for Veteran Families (SSVF), or other forms of housing in the community. Coordinating with the locale Suicide Prevention Coordinator(s) (SPC) to enhance suicide prevention amongst homeless Veterans encountered through community-based programs. Providing leadership in the community by actively participating in the local plans to end Veterans homelessness in addition to conducting and leading the coordination of outreach services to homeless Veterans. Facilitating VA involvement in the CoC's Coordinated Entry System (CES), per regulations and guidance published by HUD. Providing case management services to a special patient population that demonstrates highly complex psychosocial, health or mental health problems. Coordinating and providing information and referrals to VA programs to assist contractors in their provision of case management services. Providing consultation services to other VA staff regarding the psychosocial needs of homeless Veterans and the impact of psychosocial problems on healthcare and adherence to treatment plans.
