The Greater Los Angeles VA Healthcare Network is recruiting for a Senior Social Worker. The position is an advance practice Social Worker providing services to Veterans in Primary Care and Specialty Care at the West Los Angeles Healthcare Center. The Social Worker also serves as a float, on an as needed basis, and serves as the central point of contact for various transplant teams and completes all of the transplant social work assessments for Veterans undergoing transplant evaluations. The Social Worker is assigned administrative responsibility for all aspects related to social work regarding clinical program development, program evaluation, quality improvement activities and accountable for the social work component of the clinical program's effectiveness and modification of service patterns as necessary. Duties include, but are not limited to: Assess and treats the complicated psychosocial problems of Veterans, families and significant other; Assists with Advance Directive education and completion; Assists Veteran and family to understand the contributing factors to the problems; Coordinates with teamlet to ensure needed services are ordered post discharge; Provides supportive patient and family counseling and peer, team and trainee education; Independently performs duties while identifying and screening patients for social work services; Independently provides social work coverage in the primary and specialty care areas as needed.
