This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. Learn more The Access Center is the main entry way to behavioral health services at the Sacramento VA, comprised of a multi-disciplinary team that provides triage, intake, outreach, orientation, post hospitalization discharge follow up and high risk case management services. 1. Grade Determining elements Clinical services (100% time): a. Works with Veterans, their families and interdisciplinary team members in the assessment, planning, implementation and evaluation of treatment needs related to Veterans that present in crisis with psychiatric disorders, with particular regard to addressing situations related to risk of harm to self, others or grave disability. Develops individual safety plans. Incumbent will perform voluntary/involuntary admissions as needed. b. Responsible for reviewing all consults/referrals for Veterans requesting mental health services. Determines the urgency of the request and Veteran state, arranging for evaluation appointment, ensuring that the consults are responded to in a timely manner. c. Conducts biopsychosocial assessments and uses DSM-5 to make clinical psychiatric diagnoses. Develops and implements advanced therapeutic interventions to bring about change in the patient, using a variety of psychotherapeutic approaches, including providing evidence based therapies. d. Participates in development of a treatment plan for each patient with whom major contact is expected, with measurable goals outlined, based on relevant psychological, medical and psychiatric information and principles of treatment matching. e. Attends regularly scheduled treatment team meetings to ensure efficiency and continuity of service delivery. f. Assist patients in the preparation of materials needed for applications to various agencies for entitlements. Serve as the clinic consultant for information on negotiating the complex processes patients encounter when interacting with government and community agencies. g. Ensures timely completion of all clinical charting and documentation required by the Medical Center or healthcare system as appropriate. h. Uses diagnostic knowledge of the patient's need(s) to make referrals to a variety of VA and non-VA resources in support of the treatment objective of the patient. i. Collaboration with community, health, welfare, and service agencies. j. Complies with national and local performance measures, including timely health care screening and completion of clinical reminders for all patients. k. Completes assigned Performance Improvement activities at the program, service line and medical center level. l. Records information, in the patient's medical record, related to the patient's data base, psychosocial diagnosis, treatment plan, and progress in therapy. m. The incumbent may participate in the training of Social Workers, Social Work Associates, volunteers, and students; participates as required and requested in orientation, training and staff development programs involving Mental Health and VANCHCS. The incumbent assumes responsibility and shows initiative for own learning and professional development. At the GS12 level, will demonstrate the ability to teach and mentor staff and students in the special area of practice and provide supervision for licensure or specialty certification n.
