Social Worker (Program Coordinator)(GPD Liaison)
The Program Coordinator is administratively responsible for the Grant and Per Diem Program. The GPD program, an essential and critical part of the VHA, is vital for providing safe and transitional housing and supportive services for homeless veterans. Duties include, but not limited to: Participates in the development of referral processes for admission and manages waiver and extension requests for participants. Initiates/monitors the billing process for the per diem payments by ensuring that invoices are accurate and processed within policy timelines. Ensures that the clinical documentation, program required documentation, and program records are in compliance with national programs, VISN and local VA facility policy and procedures in accordance with GPD provider original grant documenti
