The Michael E DeBakey VA Medical Center (MEDVAMC) is seeking a Senior Social Worker-Psychosocial Rehabilitation & Recovery Center (PRRC) that will be located in Houston, TX. The incumbent functions as a Recovery Coach within the PRRC, responsible for case finding functions, outreach, screenings, initial intake, detailed psychosocial assessments, and creating a working diagnosis. Subsequently, the incumbent along with the veteran, their support system and Interdisciplinary Treatment team will develop of an individual recovery treatment plan that increases independence in the preferred environments in the community of their choice. The incumbent continues to work with assigned Veterans over time, monitoring participation in PRRC activities and progress toward individual recovery goals. The incumbent actively participates in regular PRRC recovery team meetings sharing observations and information aimed at enhancing Veteran's treatment. The incumbent implements treatment modalities by providing individual, group and family psychotherapy and measures outcomes to intervention with the goal of promoting recovery. The incumbent independently identifies high-risk patients and provides case management services. The incumbent provides crisis intervention services, seeking to address the cause as well as the presenting complaint, coordinates family conferences and serves as liaison to family members as the Veteran allows. The incumbent has a working knowledge and experience in use of medical and mental health diagnoses, disabilities and treatment procedures.
