This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application. Learn more MAJOR DUTIES ~ Provide comprehensive primary care and preventative care to an established panel of primary care patients, under the Patient Aligned Care Team (PACT) model. The type of care is served to the Veteran population with diseases consistent to those seen in the general population include, but are not limited to: acute and chronic illness, women's health, gender specific, mental health, and preventive care, along with coordination of care delivery. A major focus is in preventative medicine. ~ Function as a leader in the PACT approach in the delivery of care, which emphasizes a team-based approach to healthcare. Participate in weekly team meetings and provide innovative management ideas for patient case reviews. ~ Duties include, but are not limited to, history taking, physical examination, ordering appropriate labs, diagnostic tests, and needed consultations. ~ Formulate multi-disciplinary treatment plans. ~Perform medication reconciliation. ~ Document all medical information in an electronic medical record. ~ Other duties as assigned.
