The Medical Foster Home (MFH) /Community Residential Care Social Worker Coordinator in the Rehabilitation and Extended Care (REC) Service Line of the VA Iowa City Health Care System serves as program coordinator and is administratively responsible for the Medical Foster Home and Community Residential Care Programs. Duties include, but are not limited to: The incumbent provides assessment, treatment, and planning for a complex population of patients which reflect the age-specific, psychosocial, and functional needs of the patient (e.g., spinal cord, traumatic brain injury, geriatric with or without cognitive impairment and mental health population). The incumbent will evaluate Veterans for appropriateness for Medical Foster Home and Community Residential care. The incumbent will evaluate caregivers, homes, and facilities as well as completing regular visits and annual inspections. Demonstrates knowledge of VA and community resources to meet the complex psychosocial needs of this unique population of patients and to ensure they are referred to the least restrictive environment that meets the level of care needed. Demonstrates knowledge of program coordination and administration which includes consultation, negotiation and monitoring. Performs a wide range of services and provides technical assistance to State, local, and private agencies, and universities. Responsible for organizing work, setting priorities, meeting multiple deadlines and evaluating assigned program area(s). Has knowledge of Federal and State laws and regulations that pertain to the care of vulnerable adults in community residential homes. Performs work related to the review, monitoring, analysis, planning and assessment of the Medical Foster Home and Community Residential Care Programs. Independently deals with crises and effectively uses his/her problem-solving skills to assist the Veteran, family and staff. Communicates with entire multidisciplinary team regarding Veterans contentment with home and care received in the home. Work Schedule: Full-time; Monday - Friday, 8:00 AM - 4:30 PM; Occasionally evening hours will be required Financial Disclosure Report: Not required
