PACT Program OverallGreater Lakes PACT program focuses on individual's recovery, safety in the community, the promotion of wellness and independence of the individual in their residential structured setting, and the re-establishment to a higher level of social life functioning. These services are designed to support and rehabilitate individuals who are experiencing symptoms of severe mental illness that impact their ability to live in the community. Seventy-five percent or more of services are provided outside of the program office in locations that are comfortable and convenient for clients. The PACT program is a hospital without walls providing services for the most vulnerable members of our community. Position Summary The Vocational/Housing Case Manager helps clients identify goals that promote recovery, provides supportive housing services, and assistance accessing vocational opportunities. Individuals served by this program may have experienced homelessness or are at risk for homelessness due to substantial mental, physical or behavioral health barriers that challenge their ability to maintain employment and/or housing or live independently. Vocational/Housing Case Manager will assist clients to achieve successful independent living, obtaining/maintaining housing, and pursuing vocational/educational goals. Provides outreach, housing search and acquisition assistance, independent living skills training, and assists clinical staff with crisis intervention and stabilization. Engagement skills are crucial in this position, as PACT clients often have a history of failing to engage with the mental health system or with other positive natural supports. All services are provided under the supervision of a Mental Health Professional. Responsibilities Provides engagement services, coordinates housing plan, and assisting clients with connecting to vocational/educational resources and develops positive relationships with PACT clients. Assists participants in developing successful independent living skills. Assists participants in meeting recovery goals. Completes clinical and administrative documentation in a thorough, accurate, timely and appropriate manner. Completes progress notes on time for each service providing a clear, concise description of the clients progress towards treatment goals, being sure to document any risk concerns. Practices effective caseload management in ways that meet productivity expectations. Qualifications High School Diploma and 2 years of relevant experience. Must have a WA State Driver's License, a good driving record, a personal automobile and personal auto insurance. Must be eligible for a Washington State Department of Health Agency Affiliated Counselor Registration credential. Awareness of chemical dependency treatment strategies preferred. #LI-JG1
