Senior Social Worker - Home Based Primary Care
The Senior Social Worker practices at an advanced level in the HBPC Program which is a major specialty treatment program and component of Geriatrics and Extended Care. The Veterans in this program are home-bound, non-institutional and usually have a caregiver. The Social Worker has responsibility for clinical program development and is accountable for clinical program surveillance, program effectiveness, and modification of service patterns.
