The Caregiver Support Program Social Worker provides services to Veterans with specialized services, including those eligible for the Program of Comprehensive Assistance for Family Caregivers (PCAFC) program. Incumbent will work closely with the PCAFC Program Coordinator(s) and the Program of General Caregiver Support Services (PGCSS) Coordinator and/or designee to complete program required home visits, telephone contacts, assessments, annual evaluations, and respond to Caregiver Hotline calls. Applicants pending the completion of education or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
