DescriptionCollaborating with patients, the Faculty Practice group, community resources, providers, managed care operations, the UCLA Health System and health plan, you will implement, coordinate, and monitor options and services to meet at-risk patients' healthcare needs. This will involve coordinating with providers, Comprehensive Care Coordinators, and staff to promote quality, cost-effective outcomes and appropriate level and mix of care. *This position is 25% flex-hybrid and includes significant field work (patient homes, community and office visits with patients)In this role, you will also be responsible for:
