About the Role: The Greater Los Angeles Health Care System is currently recruiting two Senior Social Workers to join our Patient-Aligned Care Team (PACT) at the Antelope Valley VA Clinic, located in Lancaster CA. As a key member of a comprehensive team dedicated to providing primary care to Veterans, you will play a crucial role in prevention, health promotion, coordination, and chronic disease management. In this role you will provide services to Veterans in the PACT, addressing complex psychosocial problems of Veterans, families, and significant others.
